Jazzy is a Behaviour Support Practitioner, Co-Founder, and Director at Centred on You. She brings extensive experience across disability and mental health settings, supporting children, adults, and families with complex and high-needs presentations. Jazzy is passionate about delivering thoughtful and compassionate evidence-based behaviour support to improve quality of life and reduces the use of restrictive practices.
With a strong background in Positive Behaviour Support, leadership, and clinical supervision, Jazzy has worked closely with multidisciplinary teams, families, and support teams to support individuals through significant transitions, including hospital discharge, community reintegration, and legal or forensic pathways. As a co-founder of Centred on You, Jazzy is deeply committed to building a service that prioritises thoughtful practice, accountability, and meaningful outcomes—ensuring supports are not only effective, but truly centred on the person.

Kerry’s Qualifications
Kerry is passionate about person-centred support, and this is underpinned by her strong working knowledge of Active Support and Recovery-Oriented Practices.
She is committed to working with individuals and their support networks to co-design and implement practical behaviour support plans that focus on highlighting and enhancing an individual’s strengths in order to achieve meaningful outcomes in their lives.
Kerry has a special interest in working with people with a dual diagnosis of intellectual disability and mental health disorder, as well as working with individuals with a traumatic brain injury, and these interest areas are underpinned by her extensive experience working in the Community Services sector in disability, mental health, and justice, in both frontline support and management roles.

Nathalie’s Qualifications
Nathalie brings 30 years of experience in the disability sector, supporting people across intellectual disability and psychosocial disability with steadiness, respect, and genuine care. In more recent years, she has developed a strong interest in neuro‑degenerative disorders, including Dementia, and is committed to understanding the person behind the diagnosis and supporting them with dignity, patience, and humanity.
Nathalie listens deeply, notices the details that matter, and adapts her support to meet people where they are — honouring their strengths, preferences, and unique life experiences.
Throughout her career, she has worked alongside individuals, families, and teams with a person‑centred approach that prioritises safety, clarity, and meaningful connection. Her guiding principle is simple and grounded in community:
